Required Band Forms
All documents must be filled out and returned to Mr. Harrington before July 11 at 7 a.m.
This includes: Physical Form, Fine Arts Field Trip Form (HS), UIL Acknowledgement Form, ECG/EKG Form, Permission Form WITH District Transportation, and Permission Form WITHOUT District Transportation, and Handbook Acknowledgement. All of these forms can be found in the links below. These PDFs can be completed electronically.
